FIRE REPORT REQUEST

If the Tiffin Fire Department was a responding agency, copies of the fire report are available upon request in person or in writing to parties involved in the incident or their designated insurance agent or legal representative.

  • The requester will be required to provide date and location of the incident or Tiffin Fire Department incident number.
  • No fax or phone requests will be accepted.
  • Mailed requests require a self-addressed stamped envelope for return.
  • Fee per incident report is $10.00.
PAYMENT
Payment shall be received for services prior to distribution of the requested material.  
Payment by check made out to the Tiffin Fire Department
or
credit card payment in-person at Tiffin City Hall.  (Credit Card payment adds additional 2.5% fee for processing)
